New Patients

We welcome all new patients.

Practice Area

You can register as a new patient with the practice if you live in our inner practice area which consists of:

Catchment Area

If you are registered in our inner practice area and then move to our outer practice area you may remain on our list.

How to register as a new patient

Visit the surgery in person and complete an application form at reception. You will need 2 forms of identification for each person, one of which should be photo ID and one proof of address (eg utility bill).

New patients will need to complete an initial health questionnaire following which you will be offered a preliminary health check.

Overseas Visitors

If you are an overseas visitor to the UK there may be a charge to see the doctor or nurse. For further information, please contact reception.

Updating Your Details

Please let us know if you move address, change your phone number(s) or email. You can do this in person or if you are registered for online services you can do this via the patient portal.

Temporary residents

If you are visiting the area for work, family reasons, holiday or studies, you can register temporarily (from 16 days to 3 months) with Kirkstall Lane Medical Centre. You’ll still remain registered with your permanent GP back at home.

To register as a temporary resident the place you are staying needs to be within our inner catchment area. After three months you will have to apply to register with that surgery as a permanent resident.

We will pass details of any treatment you have to your GP, who will add the information to your medical records.

Important Information for Patients Living Outside the Practice Boundary

We understand that some of our patients may have moved home and now live outside our practice boundary. We are happy to continue caring for many of these patients where it is safe and clinically appropriate to do so.

However, we would like to make you aware of the following important limitations and responsibilities:

1.  No Home Visit Eligibility

If you live outside the practice boundary, you are not eligible for home visits. This is in line with NHS England guidance. Should you require a home visit in the future, you will need to contact NHS 111 for further support, as we will be unable to attend your home.

2.  Clinical Reviews and Face-to-Face Care

To ensure safe and effective care, some medical issues require face-to-face assessment, physical examination, or ongoing monitoring at the surgery.

If you live too far away to attend the practice for these necessary appointments, and instead rely solely on telephone or online consultations, we may not be able to provide the standard of care that we consider safe and clinically appropriate.

In such cases, we may advise you to register with a GP practice closer to your home, where you can more easily access the care you need.

3.  Clinical Safety and Fairness

Each case is considered individually by a GP. Decisions are based on:

  • The nature of your ongoing health needs
  • The ability to safely manage your care without regular in-person reviews
  • Any concerns regarding delays or missed diagnoses due to remote-only consultations

If we believe that continued registration is not in your best clinical interest, we will contact you directly to explain our concerns and provide appropriate notice and support.

Questions?

If you have recently moved and are unsure whether you are still within our boundary, please contact our reception team. You can also check our boundary map [please add the link to this]

We remain committed to delivering safe, effective, and accessible care and we thank you for your understanding.
